Patient Experience Questionnaire

We recognise that not everyone wants for to share their feedback formally with us and so by completing this questionnaire, you can share your experience on our service anonymously.

As part of this questionnaire, there are some non-mandatory questions on protected characteristics to help us understand if there are any groups of people we are not hearing from.

The themes and trends from the data will be used and shared appropriately to help us improve and develop the service for the benefit of other service user.

This doesn’t prevent you from providing your feedback in other ways and these include:

• Raising a concern or sharing a compliment through our Patient Experience Team
• Joining our community forum
